Integrative Care is happy to accept new patients. To schedule an appointment, please call us at 623-972-1055.
During your first visit, we ask that you bring all pertinent medical records and a list of current medications and dosages. For your convenience, please download, print and complete your forms prior to arrival.
To expedite the registration process, we encourage all patients to bring your insurance card, photo identification and co-pay.
As a courtesy to our team and to other patients, we ask that cancellations be called into the office at least 24 hours in advance.
